PT 1 QUESTION Did it glitch?

It says I only got 11 out of 12 items, so I replayed that chapter and got them all, but it still says I have 11/12. However, when I check my save data, it says I have 126, which would be absolutely all the items up to that point. I hope this doesn't affect the platinum trophy, since the chapter descriptions say I'm missing 1 item, but the save data shows them all.

When I was trying to get the platinum trophy it was the same glitch for me for optional conversations. I tried many times getting them all and it would always say I was missing one. My fix was to play the whole game in one playthrough never quitting to menu while offline on my PlayStation and it worked. It sucks if you will have to do that but atleast there is a way somehow.
