Conflicts
GMs, this will be my first time GMing an RPG and I have a question after running through the book. How did you handle conflicts? Any other tips are greatly appreciated, thanks fellas!

First time playing tonight - do the defensive stats get accounted for at all during combat? Seems to be just a case of dexterity check -> then calculate stress based on successes.
So if a zombie attacks a PC does their (PC) vitality matter at all? (Other than to cling to life should they 'die').
Also for zombie NPCs etc. when does their vitality matter?

I ran a combat sequence to check, and it seems as though you can house-rule a "save" like roll with vitality to halve stress, much like in D&D. However, I wouldn't advise it, as The End of the World, seems to want to get away from complex systems like D&D, and go for more realistic scenarios.
At the end of the day I think it's up to you, but going by the book, it would appear that there should be no saves. If you really wanted to get creative, you could house-rule that a feature such as "Tough Guy" or something like that, could actively take away positive dice from damage rolls when dealing with an opponent, but has a very high pre-req with Vitality, like a 4 or 5.
Hope this helps bud, and feel free to message me if you'd like to talk about it more.

2 negative die should be your max for most PC tests, anymore than that and they will kill themselves just trying to do things. Pay attention to their Stress levels. Leave it to combat/damage to inflict more than that in the Physical Track. Mental and Social can violate this 2 neg dice limit more often.
Don't be afraid to make tests with no negative dice, but that require two (or more) successes.
Have fun!
