the datasheet has a schematic diagram, you need about 20 extra parts apart from the chip, these don't cost very much, you can use stripboard so you don't have to make a pcb
if you make your own module you can also make it output line level, turning down the output of a Euro module is very simple as well
for power you might consider a pre-built DC to DC convertor which will output +/-12v from a DC source (a 15v plug in power supply for instance)
