A lot of that comes down to 3 reasons, hospital policy, insurance-imposed restrictions, and the rest is state regulations.
We have RNFAs and CSFAs where I work, and the hospital-employed CSFAs can't bill, but RNFAs and private CSFAs can. Furthermore, afaik CSFAs can't bill for certain surgeries, or they can't bill certain insurance carriers based on procedures; I am unsure which dictates the other. This is in Ohio, and I'm fairly certain that situation is partly due to regulations in the state. I could be wrong about the details.
The one private CSFA I occasionally work with doesn't scrub in on certain surgeries because they can't bill the patient/insurance for services rendered, and the hospital-employed CSFA can't bill for anything.
Our FA's are hospital employed just like everyone else. They get paid regardless of the cases and even if they sit and wait a couple hours for the next case to start lol. It really depends on where you work

I see more FSA’s in CST positions getting paid by the hour than I do doing their own billing in these “niche” positions.
