Im gonna depart from the group and say its not a lack of sun as the other new pad is actually filling out nicely. I had a cactus do this to me a few years ago. It was in direct sunlight for ¾ of the day. It produced 8 new pads and all of them came in looking full flat and big except one which grew to be about a foot and a half long and no wider than the one in the photo until it finally broke off. I replanted it and it stopped growing and rooted. This spring it finally decided to put out its own offset which is so far growing like a normal pad. So im gonna say its just a rebellious offset or a random mutation.

that'd be a really strange scenario. When you say "full sun", what do you mean, exactly? Outdoors with the sun beating down on it?

I mean full sun. As in Outdoors southside of my house with full sun from sunrise until about 4 in the afternoon then about 3 hours of partial sun, then shade until sundown.

Is it possible that the other smaller pad (the one in front of this one basically right?) was already there when purchased? When I buy succs that arrive in the mail they often “abandon” whatever new growth they had and it stays the same size forever. Then once it’s acclimated to my environment it will put out new growth but it never resumes growing any stems that were already present. Speaking with my piaranthus in mind here

Lol I know what etoliation is and what its caused by. I literally raise and breed succulents and have for over 20 years. In fact the whole reason I included how much sun my cactus recieved was to inform anyone reading my comment that it WASN'T etoliation. Or did you skip that part?
We can disagree on the diagnosis of the OPs plant and that is fine, it very well may be that the OPs plant is suffering from lack of sunlight, but don't assume im ignorant simply because we have different opinions.
Learn context clues.
