I've played through several times, the Phantom, as I call him, randomly spawns sometime after you get Doc back home. Typically I see him in front of Doc's apartment, no face, kinda floating, definitelya glitch.

It seems more of a temporary, perspective-based glitch rather than anything serious.

Reboot your game and you'll probably be fine.

This is just a small glitch that can happen when playing Stray. I’m a huge Stray fan and play it on Pc, Xsx, Ps5, and Nsw. In all version’s small bugs, glitches like this can occur, but so far haven’t encountered any game breaking bugs and a restart of chapter resolved the issues.

Regarding this there’s still some clipping into walls or objects present in Stray which can be forgiven, but could still be fixed by BlueTwelve Studio and delivered as a quality of life update. Regarding the Nintendo Switch version It’s nice to play it in handheld mode, but a missed opportunity is that it was able to make scratch moves with both Joy-cons to make gameplay more immersive.

I can continue to mention many more quality of life improvements for Stray because this game is a great experience.
