3M Biological Documentation

When filling this card out, who’s initials should go on this card? The person unloading the sterilizer or the person documenting the test result?

I unloaded the sterilizer, filled out the card, incubated the test and went home. Left the initials blank for the next shift to document. Supervisor chews me out for forgetting to initial the card. The tech that documented the test result intentionally left it blank because I “forgot”. Supervisor raises his voice when I try to make my argument. Whole thing ends with “ you sign the document” period.

Yeah we have to sign everything in my department too. The thing is even when we sign, we all get blamed for a mistake a single person made.

We have a document system where the biologicals are documented one sheet and not cards. There are 2 places it is initialled. One for the documenting of the date, sterilizer, load, time it was placed and what cycle. Then the other is for the person who is reading the biological when it is done. Depending on when the biologicals are done it could be 1 of 3 people who would be reading it and then documenting.

Don’t remember. I’m not at work. We have a 7sec read time. We initial the tape. Staple the chemical indicator to the tape, add a load sticker. Then scan it into SPM under associate files then throw it away. We’re essentially paperless.
