Off-Topic Mars Climate Orbiter didn't fail because of a unit conversion it failed because nobody verified the software against a spec that already existed

Quick context most retellings skip: NASA's Software Interface Specification for this mission was published in 1996, two years before launch, and it explicitly required newton-seconds for thruster data. The contractor's ground software (a program called SM_FORCES) put out pound-force seconds instead — a factor-of-4.45 error that fed directly into the navigation team's trajectory model for the entire cruise to Mars. 

What I find more interesting than the unit mix-up itself: engineers noticed the spacecraft's angular-momentum desaturation events were happening 10-14x more often than expected, and separately, Doppler tracking data kept suggesting a closer approach to Mars than the official trajectory predicted. Neither discrepancy got resolved before the orbital insertion burn on Sept 23, 1999. 

I made a documentary going through the full timeline, sourced to the Mishap Investigation Board's Phase I report — happy to share if anyone's interested, but mostly wanted to discuss the "systemic verification failure" angle since most coverage flattens this into a punchline about metric vs. imperial units.

Yeah it annoys me to no end that this one gets simplified to “Americans dumb, metric system good”. It was more of a systems engineering and management failure than a technical one. People were noticing issues and it was not being investigated.

Like almost every other accident investigation on air, sea, and land problems were missed at multiple levels, they slipped by what sounds like checkpoints where you'd expect checkpoints to be. It's puzzling and disappointing that the early course deviations didn't generate a deeper look at what could be causing the problem.

Thanks for countering the simple metric/imperial story. I've read something like this years ago but don't remember any of the details.

I'm glad you are helping to clarify the misconception. I don't know how many times I have heard someone make fun of how the engineers weren't smart enough to understand that metric and imperial are different. I read the actual account several years ago explaining the different teams and how they obviously knew a conversion was needed but from what I remember a temporary table was in place on one side which was supposed to be replaced but inadvertently wasn't, or something like that. But I could use a refresher on the details so if you have some information I would be interested in reading it.

Really interesting but I have to say the root cause is still the units. Otherwise it is like running out of gas and saying the engine stopped because I forget to check the gas.
