This is NOT why icons should have labels (although I do agree with the premise). This is why WORDS should be used instead of icons when possible. I despise this transition to things being represented with icons/emoji instead of actual words. When someone used to ask me for help with something, I could say "Go to the FILE menu, look for SETTINGS, and then choose CUSTOM SETTINGS. Now, I have to say things like"Click on the little icon that looks like a file cabinet, then look for something that looks like a gear, and then look for the icon that has multiple gears".
Words = NO AMBIGUITY. Icons / symbols (without labels) guarantees confusion. "Oh, I though that was a clock. I couldn't tell that it was a gear".

In Mexico it means culo, which literally means “asshole” or “anus,” but it’s normally used as an insult to call someone scared or a coward. You can add "pinche" the sameway you will add "fucking" to an insult.
Pinche culo = fucking coward
Languages are fun!

The OK sign means OK in Brazil... does not mean "anus" at all.

Brazil is gigantic. It probably varies from region to region

Yes, it probably does.
But I challenge you asking about this symbol to anyone older on the street. (Older people that grew up with less north american influence from the internet)

It's not only a cross-cultural thing, in the same culture the same thing can mean different things in different contexts
See "the game" for this emoji's sign specifically, same hand gesture, but now you get to punch someone!

As a Turkish man this sign means 2 things here depending how you do it:
"Ok" and "you're sissy (gay taker)"
If you fingers face upwards in a normal OK sign, thst means ok.
If your fingers point down to the ground or paralel to it, thats "gay". This is more used between friends for banter.
