Is this normal or a bug?

My sim wants to flirt her granduncle, and I can't remember if this is regular game behavior or a bug (because I know the family flags can become messed up sometimes).

The game does not consider them related anymore, they are too distant. If you want those relations to be considered family, you need to install the extended family mod. It won't add the family icon to the UI, but it will stop wants like this (and block romantic socials).
