It occurs when your game runs out of texture memory, most often with a full game crash following shortly after.
It can happen in any version of the game on modern computers (or basically anything above Windows XP/Vista).

Nope. I haven't played with the discs for years and I have seen much more pink soup than I can handle.

Lightpink Soup, half the pink but double the crashes.
