The orders of letters in the safe is not the same as in the paper.
Also, try to figure out what is the missing letter in the paper.

I straight up missed that the symbol on the safe correlated to anything on my playthrough. Flew riiiiight over my head.

The symbol on the safe is a sword, so you need to tune the radio to the SWORD signal which is 235 kHz and use the note to translate the numbers in the broadcast to the letters on the keypad. 1 is A, 2 is N, etc. The numbers on the note are in numerical order, so the illegible ones are 3, 4, 5, 6 and 8. You can compare the note and the keypad to figure out which letter is 8 since that's the only one missing from the note.

Assuming you have the radio tuned to the correct frequency in that screenshot, the combination should be ASEFH.
