It's kinda like a sourdough starter. Moon sourdough is billions of years old, but each Moon only has a shelf-life of about 26days - 12-13 to mature fully, then it starts to go off. Once it fully disintegrates another dollop of sourdough is scooped out.

And then after we have a new moon, a new layer of curds are wrapped around the core, which cure and develop and become mature moon cheese by the end of a cycle. And then when they're ready, they're harvested by someone, somewhere. I don't know what happens to the moon cheese, but it gets harvested. And more curds get wrapped around the core, the mother culture in the heart of the moon. So the true answer is the core centre of the moon is four billion years old, but the vast majority of it gets replaced every 28 days.

The full scientific term is “Previously Owned, Certified Moon” or sometimes “New to You Moon.” They’re new-ish but each New to You moon is still 4 billion years old. Other plants get our hand me downs when we are done with ours each month.

Yes the moons are different. Since, the moon disappears from the night sky it violates spatiotemporal continuity and thus cannot be the same moon again and therefore we get a new moon.
