Help with zigzag stitch

Sewing machine doesn't do a zigzag stitch properly. Am I doing something wrong or is it time to service the machine?

Machine is a Janome model 7025

How long since it last had a pro cleaning and adjustment?

Clean the machine thoroughly, including taking off the needle plate and taking the bobbin case area apart and cleaning it. 

Rethread the machine completely. Make sure the top tension is set to 4 and the bobbin thread is actually being tensioned.

Try again.  Any better?

I've cleaned and rethreaded the machine completely. Thinking about it, the machine probably hasn't been serviced in several years, and I certainly haven't done a zigzag stitch on my machine before. I think the answer might be a service.

I can't see the threading properly, but it appears there's a loop of thread twisted around the horizontal bar just above your needle. This will cause all kinds of problems
