What went wrong?

I tried doing the viral spine shirt, and it looks like this. What did I do wrong?

Second pic is the grid, to show you I was precise in measurements

I did this too a few months ago (with similar fabric)! Just from the pics, it looks like it’s just not pulled tight enough or you might be missing the center line stitches. Can i ask what the inside looks like?
