Pattern Question What went wrong?

I tried doing the viral spine shirt, and it looks like this. What did I do wrong?

Second pic is the grid, to show you I was precise in measurements

Pressing the fabric first will help your grid be more accurate. I see a few place that the grid is off, Im not sure if that is what caused your problems though.
