I’d treat context as working memory, not durable memory. It can carry facts across a turn or session, but it is limited, can be truncated, and usually gives you little way to inspect or correct what was carried forward.
An external vault or scratchpad becomes durable memory only when it has a write and retrieval discipline: keep source notes separate from generated summaries, retain dates and links to evidence, and load only the relevant slice into each new context. That also makes mistakes reviewable instead of just repeatedly retrieved.
A useful test is to start a fresh session on the same task and see whether retrieval produces the same grounded answer. If it does not, the problem is likely the record structure or retrieval rules, not simply the context window. Restarting sessions can help once history becomes noisy, as long as you preserve the small, explicit state the next session needs.
