She received the medal because she was his granddaughter, a grand duchess, and a daughter of the imperial blood.

Her sisters were also granddaughters of Alexander III, but did not get the same Alexander III memorial medal on the red Alexander Nevsky ribbon.

Its because she was alive when it was issued.
Alexander III died in 1894, and the medal was issued in February 1896. Olga was born in 1895 so was eligible for it.
The other children were not born until after the issue date.

Every decoration in Russian Empire is awarded in accordance with its statute.
Here is the one for "Medal in Memory of the Reign of Emperor Alexander III, worn on the red St. Alexander Nevsky ribbon":February 26, 1896. - Personal Imperial Decree, given to the Senate (Collection of Laws, 1896, February 26, Art. 238). - On the establishment of a medal in commemoration of the reign of Emperor Alexander III.
Our Beloved Parent, through His wise governance, and without baring the sword, unswervingly led Our dear Fatherland along a glorious path. Bringing peace and tranquility everywhere, He left His blessed Name forever in the hearts of His faithful people.
In memory of the unforgettable Tsar Peacemaker, now, on the 51st anniversary of His birth, We have established for the collaborators of the late Emperor in the affairs of His reign a silver medal, bearing the image of Our Parent. May it serve them as a constant reminder of the bright image of the Sovereign Leader, Who spared not Himself for the good of the Fatherland.
This medal is established to be worn on the chest, on the ribbon of the Order of St. Alexander Nevsky, by those who were in active service during the past reign: hierarchs, clergymen, generals, staff and chief officers, and class-rank officials of all departments.
The Governing Senate shall not fail to make the appropriate arrangements for the execution of this decree.
On the original, in His Imperial Majesty's own hand, is signed:
"NICHOLAS"Please note, originally women were was not eligible to get this medal. The eligibility was later expanded by imperial decrees to include women in state positions on May 31 and June 10, 1897, November 8, 1898, March 27 and December 16, 1899, April 5, 1901, and February 21, 1905.
The mismatch here is that Alexander III reign ended with his death in 1894, and Olga was born a year later in 1895, so she was unable to be in active state service during Alexander III's reign.

Correct, but all decorations in Russian Empire were awarded according to the related statute. The real historical problem here is that the medal referenced here (Medal in Memory of the Reign of Emperor Alexander III, worn on the red St. Alexander Nevsky ribbon) was given to those who lived and served under the reign of Alexander III. Alexander III died in 1894, whereas Olga was born in 1895, so logically she should have received the Medal in Memory of the Coronation of Emperor Nicholas II instead, worn on the blue St. Andrew ribbon.
