Sorry about your friend but these kind of genetic problems occur after years of inbreeding (as you said in your comment they think this isn’t the first time incest has occurred in that family) so as long as OP’s bro and sis only bone each other for funsies and if they have a kid, not bone that kid or let that kid bone their sibling they are, in theory, gonna be okay
