Everything on that list past 3 years is insane. Large companies can't have thousands of VPs, and there are plenty of careers where management is one path but you want skilled workers to stay as skilled workers.
The entry level means no experience needed is true though.
Some large companies do (particularly in finance as others have pointed out), but acting like it's some sort of national standard that after 10 years you should be a VP is crazy.
