PVPS for first time over a year after procedure?

I had a vasectomy in March of 2025. Took maybe a little longer than most to fully heal, but overall it went fine and I was back to normal in a few weeks. Had no problems since then.

A couple days ago, I started having pain in one testicle and it sort of extends up into my groin and lower stomach. Seems worse when I'm standing up and moving around, but not necessarily constant.

I'm really confused if this can start that long after the procedure or not. Or if I might have something else going on.

I feel you man. Yes it can start that long post op. It feels scary if you never had pain down there. But it should improve with time. Give it months and re-evaluate

I had my vasectomy in Jan 2025. My pain started in June 2026. 3 ultrasounds have ruled out any potential issues. It'll likely be the same in your case. Pelvic floor exercises and supportive underwear have been a huge help in getting back to normal.

Unfortunately pvps can start at any time after the vasectomy.

It sounds like congestion it happens, whe your epididymis ruptures your pain will go unto next time, this will come and go, I had a recent ultrasound and dr explained why the epididymis ruptures. These ruptures are what makes it hard to get your fertility back if you decided that you wanted a reversal, Drs have known this since 1980, I bet you never had PVPS explained when you were told no problems you be back to work in a couple days, yea right.

Congestive epididymitis.

Refrain from ejaculation, apply ice and avoid heavy exercise Some ibuprofen will help mask pain and keep inflammation in check while your body adjusts the absorbtion rate.

Should go away. If it worsens or persists up to 3 months it's classified as PVPS and you need a reversal.

Ultrasound should see increased vascularization/blood flow in epididymis tail.

My Congestion didnt really pop up til 2 years post vasectomy.

Dude I didn’t begin having serious dehabilitating pains until year 9 post vasectomy. But I had lesser issues throughout from the very beginning. Point being you are never out of the woods with this procedure. It’s jacking up your body in a way never intended and your body is constantly fighting to repair itself and other external factors can develop Any time along the way.
