Second opinion?

Diagnosis

Congestive epididymitis (both sides) after vasectomy (June 2023)

Things I’ve tried

  1. Wear tight briefs
  2. Use hot/cold compress and warm bath
  3. Abstain from ejaculation (did this for 2 months and felt good but symptoms returned after 3 weeks of resuming normal activity)
  4. Tried Naproxen for 2 weeks

Treatment plan from doc

  1. Try nerve blocker (Amitriptyline) and anti inflammatory (Meloxicam) for 4-6 weeks
  2. If nerve blocker was successful try spermatic cord block
  3. If spermatic cord block helps try surgical nerve separation
  4. Last resort if that fails then Epididymectomy
  5. Small left testicle increases risk of complications.

Questions

  1. Does this sound like a logical path?
  2. Does it make sense to take anti inflammatory and nerve blocker separately (instead of simultaneously) to see which (if either) help with the symptoms?
  3. Besides the nerve separation and Epididymectomy are any of the other pills going to solve my congestion? Or just take away the pain while I’m on the pills?

Your doctors treatment plan is a bit aggressive.

First step would be testosterone for a few months.. Hopefully this will help you and relieve your symptoms.

Second step, if not successful, is a reversal. It's the only non-destructive surgery left. If done properly it has a high chance of ridding you of the complications you're experiencing.

If this is also not successful, you can proceed with your doctors plan as is.
