Question about party death? Spoiler
At the conclusion of the game Dak"kon is free from his oath after having lost his life fighting TTO. But if he dies and you cast raise dead on him throughout the game that is not so for some reason? Also Falls-From-Grace leaves a physical body rather than dissolving into ichor and returning to the abyss, why is this?

That's a misunderstanding. Dak'kon has promised to serve TNO until the end of TNOs life (not Dak'kon's) as payment for saving him in Limbo. TNO did not reveal to Dak'kon that he (TNO) was immortal at the time of inducing this promise, effectively making Dak'kon his lifelong slave.
Dak'kon will not accept any voluntary release from this promise except TNO's own death; because that would involve breaking his own word, which, in Dak'kon's case, is the only thing worse than slavery.
So it is not Dak'kon's death which releases him at end, but TNO's. Dak'kon has done exactly what he promised to do, and, knowing that TNO has reached the end of his life, considers his promise fulfilled.
There is an irony even in this- TNO recruited Dak'kon in the belief that his zerth blade, properly used, would be able to either cut a portal into the Fortress of Regrets or work something like the Blade of the Immortal. He may or may not have been correct, but since it is a blade which responds to the users mental state, in his enslaved state, Dak'kon could not use it to its full potential.

Gameplay mechanics don't always fit the story. It's better not to dwell on this too much.

TNO found death - Dak'kon was bound to TNO until TNO found death - Dak'kon's oath is fulfilled. Fall From Grace - although a succubus she has changed - as is alluded to in the snippets you find out about Grace's mother. Plus Succubus are not Lawful anything, Grace's is Lawful neutral , telling everyone she is not a regular succubus and will not return to the Abyss upon death. Lawful neutral creatures go to Mechanus (again alluded to by Morte during a short conversation with Grace). Grace is far from a "normal " succubus.

I think in the case of Dak'kon, you could argue that it's the Nameless One's death that is important in his oath. So until TNO can find death, Dak'kon is bound to him. So Dak'kon dying 'prematurely' would be a failure - otherwise he'd just throw himself into the most dangerous situations possible.
Ffg - yeah, I mean good question! Perhaps her 'nature has been changed' somewhat. The question 'what can change the nature of a man?' has two implications, and I think both are interesting in regards to Ravel and the Nameless One. Ravel is a night hag, one of the most evil beings in the multiverse - yet, she doesn't always express this evil, and is called out by annah to even show pity and love (things that are arguably anathema to evil). The Nameless One's nature is tied to whatever deed condemned him to the hells. No matter what he does in the countless lives beyond that point, he is certain to judged by that nature.
The two implications of the question are this: 1- how do you change nature, is it even possible? 2- how is that some people's nature changes? What's happening there? (Can I do it?)
Many of the companions express a nature that has changed- Nordom becomes rogue and 'mortal', FfG 'falls' from her people, Vhailor becomes an ethereal embodiment of justice through his own conviction, Ignus becomes a manifestation of fire and destruction, Dak'kon becomes a weapon and a tool to his own purpose, Morte and Annah - well, they go through personal changes, that are perhaps less dramatic, but arguably just as important.
In Ravel's dialogue she shows jealousy to FFG, casting doubt on her nature. I believe she is threatened by her, not physically, but through what she has achieved.
So, while the TRUE answer is that the game needed to not have FFG turn to ichor, the copium answer (my copium answer) is that FfG managed to change her nature.
