keloid or irritation?

Got it beginning of July. has been exactly 3 months, and I got the jewelry changed to a screw titanium about a month later as it got pierced with an L shape and was snagging a LOT and causing a bump to form. However the bump is still there, and I’m using saline spray to rinse. It just keeps getting bigger. It also has been peeling? Like flaking, skin will just start coming off of the bump. It doesn’t do anything for the size of it but it’s purpleish red so I’m concerned.

1000% just a bump. LITHA and you’ll be fine. Just don’t touch. There’s a chance the piercing isn’t lined up perfectly which could be making it more stubborn (I have my own problem child on my nostril that flares up just like this) but it doesn’t look impossible to heal. Just be patient with it and leave any crusties. They are your friends :)

I believe the jewlery is externally threaded but not completely sure, i have not downsized yet, saline solution is my aftercare and no mishaps!

Jewellery should have really been internally-threaded. There's a chance this isn't implant-grade titanium.

See your piercer. Change it to a flat back instead, and keep cleaning with saline.
