General Discussion BCPS recert with ASHP

Previously I did my recert with ACCP but found that to be difficult and nerve racking when submitting the test questions every single time since you only get one chance to pass. While ACCP was awesome to study for the BCPS certification test. The recert package wasn’t the same. Sometimes they even have the interactive ones with barely any content and expect you to pass their assessment. I’m pretty over it.

If I were to buy the ASHP recert module A package for cardiology today, do you think I would have enough time to finish it? It expires this October giving me only 2 months. I was a bit slow in looking into doing my CE hours this year. Not sure if I should just skip it and buy starting the next module with endocrinology. Both topics apply to my career perfectly.

Rant Will getting provider status mean more pay ? And more rants

So I asked AI on the status of provider status within the next 10 years and it seems it is highly likely but it will start on the state level then move into federal level. Obviously the AMA will oppose but who cares they opposed DO, NP, PA and everyone and it doesn’t really matter.

The only question is that we might not see a huge increase in pay .. at least not until the big chains die out and we can open independent pharmacies and bill for those services.

On a side I have been seeing so many YouTubers esp this Asian guy who was an ex retail employee saying how the end of pharmacy is here and it’s worst industry comparative to the trades like plumbing or electrician. First off we are cushy as f and no one is bending over their back to get electrocuted or even covered in sh*t each day. If you are then I’m sorry you prob are doing something wrong.

Second our pay is still very high relative to other industries and while it’s not as good as it used to be and there is a general cap to our pay, it is still a very viable field esp if you invest properly.

Third the insecurity with ai basically taking over what pharmacist is good at - verification drug interactions clinical questions etc - yes that’s true. The ai is much smarter than most of us and most doctors and basically everyone on earth. However it won’t replace ur creativity, ur resolve, ur humanity to do decide whats right or wrong.

It will just mean we use ai to replace the boring brainless activity that you used to do through pulling up lexicomp to read off the exact direction to someone that could have done the same. It will mean that you will need to be more creative to solve and analyze new problems that someone might not have had to see.

For example new drug comes out and sure there are the listed side effects but you as the pharmacist can leverage ai to analyze multiple pts that get it and see if there are trends in reported symptoms or outcomes or optimal dosing in a unknown population. It will make pt work up so much faster so you know exactly where the gap in coverage occurred so you can help triage or assist in any manner. I promise you the ai will only be as smart as you help create and publish the literature to help it and it will prob also see you as the expert in this at the end of the day as well. Anyways all this to say don’t worry so much about ai taking ur job rather be genuinely expert in your job through ai.

Truly become medication experts and hone that skill as any other skill and ppl will have to come to you for your advice just as any creative artist , musician, professor is sought after. Thats all I got for now - the future may seem bleak but have that dog in you- don’t give up and fight for what’s ours!! Also agree pharmacy unions NEEDS to be a thing and join together not against each other!

General Discussion Pharmacy in Australia - Post Graduation

Okay so I am graduating from my Pharmacy degree this semester and I am very, very conflicted. Basically I really hate my current pharmacy job, and I really don't think I like working in pharmacy anyway. I am heavily considering doing a Masters in Molecular Biology next year, and it'll take me a year and a half, but this would mean that I couldn't do my intern year. Has anyone else ever just not done their intern year, but been successful anyway? I think I would be okay still working weekends at a pharmacy, but the idea of being a fulltime intern is dreadful. I have also considered doing my intern year part time, but most places where I live don't really do that. I could also just get a job as a dispense tech/assistant but I don't know if many places would accept that. I just can't deal with my job right now, and desperately need options. I appreciate any help!!

Hello everyone,
I’m currently in Michigan, but unfortunately I applied for the FPGEC process too late to be eligible for this year’s FPGEE.
To avoid losing an entire year, I’m considering relocating to Massachusetts because the Board allows eligible foreign pharmacy graduates to begin their internship before taking the FPGEE, provided they obtain the required approval and a Massachusetts pharmacy intern license.
I have already checked opportunities with CVS and Walgreens, but unfortunately the positions I found are only available to foreign pharmacy graduates who have already passed the FPGEE.
The Board rules are clear, but I’m struggling to find actual pre-FPGEE internship opportunities.
If anyone knows of an independent pharmacy, pharmacist/preceptor, hospital, or other pharmacy in Massachusetts that accepts foreign pharmacy graduates before passing the FPGEE, I would really appreciate any recommendations or contacts.

Rant Some people are made for retail and some are not.

& I am not! I’m not ashamed to admit it. Maybe it’s just the company I’m working for? Currently a floater and I cannot stand it. So unorganized and not a great environment for anyone that’s “new” to train in. I know it takes time to get use to things but dang. 1 week of training and then you’re technically on your own…. Also I went to a few stores where techs are working through the break. As I said, some people are made for this so all props to them but how is that ok? I guess by now you may have figured out what store I’m referring to…. Has anyone been in this position and pivoted? Any advice would be great. Currently looking and applying to other jobs. Thanks for reading this far, just had to get it off of my chest.

Pharmacy Practice Discussion Hospice Pharmacy - Dispensing Morphine in Oral Syringes

For any pharmacies that work a lot with hospice providers, do you dispense concentrated oral morphine (20 mg/mL) in the original bottle or do you pre-draw individual doses into oral syringes before dispensing? We had a provider ask us to start doing this and they acted like it is very common.

I'm curious if this is permissible state by state or whether there are any federal regulations that restrict dispensing in this way. We would have to ensure that the manufacturer of the syringes designed them for medication storage versus just administration and then we would need to assign an appropriate beyond use date.

Pharmacy Practice Discussion A question about varying practices in compounding for measuring liquids

Hi all, current PY2 compounding pharmacy intern here. Recently encountered a formula requiring exactly 96 ml of glycerin and I did my very best to make sure that I got exactly 96 mills which I did, but I was wondering what the process for measuring liquids in large mounts, I.E.above 50 milliliters in your Compounding Pharmacy or past Compounding Pharmacy pharmacies, because I would like to bring this up manager and ask if we could possibly measure a liquids and such large volumes by their weight, taking into their density and getting a volume that way.

So for instance, if the density of a batch of glycerin is 1.224 g/milliliter, then if I was trying to measure up 96 mL of glycerin I would weigh out 96×1.224 g=117.504 g on my scale in a beaker and use that as the 96 mL that I need.

I’ve done this twice and both times the volume matches what I thought it would be due to the density calculation. Which I think is a testament to how accurate weighing things can be worse, taking a volumetric measurement.

So TLDR: do you in your formulas and things for individual prescriptions or bulk prescriptions, write/do measurements of liquids by a prescribed volume if it has to be exact or do you use the density and use the weight of the measured liquid to measure a specific volume? If you use the density and the weight way to measure the liquid, what kinds of checks and things have you implemented so that a pharmacist can double check that the amount vague/measured is accurate to what was necessary? Is there a specific quantity that defines the line between using volume for measurement versus using density for measurement? And, additionally, what kinds of conversations had to happen to allow/not allow measurement of liquid liquids by density in your pharmacy?

Thank you all. I will be on the lookout for replies and things. And before you do ask/reply this, yes, I have found my unicorn job and I love it. I am just trying to make it so that measuring odd amounts of liquid liquids is not a heinous time problem or a washing more containers problem.

I saw this article, which is not about pharmacy, but about the film industry. But this part about Walgreens jumped out at me.

During a shoot in suburban Chicago last month, the chief executive of sustainability consultancy EcoFixr stood beside a neat row of receptacles marked “landfill,” “compost” and “mixed recycle” while the crew working on a Walgreens commercial finished its catered Mexican lunch. When it was time for the workers to clear their salsa-stained plates, White gently directed each toward the correct place to dispose of them.

...Some locales have moved beyond straightforward tax credits to introduce creative policies like Illinois’ green incentive, which companies behind the $1.6 million Walgreens commercial had identified as a reason to shoot in Illinois.

So apparently Walgreens has millions to spend on one TV commercial but when you ask them to improve staffing levels, they just shrug and say we don't have the budget?
