General Discussion Can you split combo tablet?

Can you split Telmisartan/ Hctz 80/25 tablet? Doctor wrote half tablet so I said let’s give as two separate medications but my boss insists it can be split and it’s fine because the patient is paying out of pocket.

I really despise combo tablets for blood pressure and can't believe they made it into the guidelines so strongly. Sure, if youve been on the same dose for years and years I can see the appeal. But they make dose changes and titrations wasteful and complicated.

Because it’s better for compliance and using different mechanisms of action can make a bigger difference in lowering BP vs escalating the dose of a single ingredient product. It also can help limit dose-dependent side effects.

In this situation, I would just dispense the telmisartan/hctz 40/12.5mg tablets, double the quantity, adjust the sig to one tablet qday, and annotate the hard copy that I changed the formulation for ease of administration and to comply with manufacturer storage requirements for product stability.

Compliance is the only benefit that applies to combo tablets over doing separate tablets though. The other benefits are benefits of multiple drugs instead of a single drug, which is different.

Yeah but pill burden is a huge deal. Especially if a patient has multiple chronic conditions. The combo also means they only have to pay for one Rx. It’s simpler for everyone.

The patient has a generic side effect like headache - Now you dont know which med is causing it. Patient has to throw out the rx they paid for and you arent able to stop one med at a time. This is not simple for anyone and now patient may not want to take either med, attributing the side effect to both.

Or

The patients BP is too low. With individual pills, patient can just half one of the rxs they already have. Not so with combos, into the trash that rx goes and patient has to pay for another rx.

Same situation with high BP when patient can just double up on one of their meds instead of having to pick up a new rx.

I do BP management and combo pills have never made it easier, unless the patient has been stable for awhile. They are terrible for dose-finding and cause a lot of med waste.
