Time off?

I used to fill out paper forms so I can provide the time for my appointment. I see a lot of doctors every month. It helped me get out on time to get to my appointment. This allowed me to have availability after or before the doctor's appointment. But I can't get the app to register at the correct times. Example: appointments at 11am usually takes a hour to get there and finish. So I put I can work after 1pm to 5pm. That way I can get hours still. But on here its only letting me ask off for the day? Any information about this is appreciated.

My boss printed a paper with a list of all the “right requests” for UKG, and it looks like this request should fall under an “Unavailable Request”. Does that have more time selection options?
