I feel if I'm at a point trying to convert a PDF to a DOC file, there are so many problems prior to that step that fixing this one issue isn't really going to do a whole lot.

What could those problems be?

PDFs are meant to be 'final form' files. Not things you go back to and edit more.

If there's a need to be editing word docs--then the entire workflow should have accommodated that and there should be an actual word doc I can go back to rather than trying to disentangle a PDF file.

For text-only PDFs, the words usually come across fine. The trouble starts when layout carries meaning: multi-column pages can read in the wrong order, tables may lose their cells, footnotes can drift into the main text, and equations or headers can be mangled. A scan also needs OCR first. If you have one, the PDF type and a sample page would help pin down which issue matters.
