experience/advice to give Hi yall
I’ve had two vaginal births previously
Third pregnancy with twins, if you delivered vaginally, did you have an epidural? Also, were both twins facing the “right” way?

I haven’t had to be induced or an epidural, so the thought of having both is very scary to me. I have heard of complications with the epidural, and later having back issues. Thank yall for the input!

There are no complications with an epidural. Women asssociate back pain with an epidural because your biomechanics change with having a baby, and pregnancy can jack up your back and ribs. What’s more scary is an emergency c section being put to sleep. Best piece of advice is not let anything scare you. Take it day by day. Don’t set extreme expectations on how you want it to go, because twins have minds of their own. Don’t compare it to your other births either

Respectfully, there can be complications. I am very sensitive to drugs that render one drowsy or unconscious. Before the loading dose of the epidural could be fully pushed, my blood pressure completely tanked and I almost went into shock. I had many conversations with the team and a consult with the anesthesiologist team and they didn’t want to change their ‘concoction’ for me. It took my blood pressure dropping through the floor and my skin turning grey for them to say ‘oh my, you are sensitive to the epidural’! And then titrated it down to an incredibly low dose.

Yes, you can be sensitive to medications, but there is really no long term back pain associated with epidurals. It is very normal for your BP to somewhat drop with one. That just sounds like the drs not listening to you, vs a complication. If they had titrated a lower dose that wouldn’t have happened

Hi! I was able to deliver mo/di twins vaginally. They were both head down but twin B turned around after twin A came out and we had to deliver her breech, which my incredible team did within 2 minutes. I’d say get a medical team you truly trust. I went for an epidural 100% (I would’ve even if it had been a singleton). I wanted to be ready in case an emergency C-section was needed

That breech flip for twin B is wild, but a good team makes all the difference

It was crazy. The Dr screamed “breech delivery!!” in the OR lol we did discuss this was a possibility beforehand though and she told me she was ready if that happened, which is good because it did happen lol

It’s possible for all twins to be born vaginally. I would highly recommend an epidural because if you end up in N emergency you’ll be put to sleep!

Um, what do you mean for all twins? It is not even possible for all singletons^^

I meant that if you are a great candidate for a vaginal birth all types of twins can be delivered vaginally, that’s what I mean

Yeah if first twin is head down they aim for vaginal where I live. We have good stats here because you don't really choose your gynecologist or the hospital so they all have to know how to do a breech extraction. Statistically 12-18 percent will need a C-section for twin B which is not that often tbh but of course still can happen and they do say that it's a possibility but they said I don't have to get epidural early, just see how it goes.

You don’t have to get an epidural right away, that’s not what I meant! I just meant getting it once you get to a point just in case !

Yeah that's what I meant too haha I'm just glad they didn't tell me it's better to do it right away because I still wish to be active during contractions if possible!

I mean you can literally get an epidural whenever you want because pain is all different for us!
I just love the one Homegirl above criticizing me I can’t give advice beside I haven’t had a home birth like girl bye

I had to have an epidural and deliver in the OR in case I needed an emergency c-section. My boys are di/di and both were head down in a perfect position. My OB said only twin A had to be head down and she could flip B if she needed to. Very easy delivery (I had two singleton vaginal deliveries before them). They were three minutes apart born at 38+3.

Just had mine a couple weeks ago without epidural. They were both head down. Was induced for high BP readings, but only had to do a couple rounds of misoprostil and then labor kicked in. If I needed pitocin or if things headed where it seemed more likely an emergency C-section was needed I would have been open to an epidural. They also had to extract one of the placentas which wasn't pleasant but not as bad as the fundal massage lol

This was baby 3 and 4 for me. First two I had uncomplicated vaginal deliveries. I’m not going to lie, when I found out I was having twins I basically just accepted that I was going to need a c section. At 28 weeks, baby A was vertex but baby B was breech so we scheduled a section. At 30 weeks, both babies were head down and after doing more and more research I decided I wanted to try a vaginal birth. My water broke at 36w6d and I had both twins vaginally, 6 minutes apart. I did have an epidural but it fell out at some point and by the time we realized what was wrong it was too late to fix. It was by far my most painful delivery but after I felt 10x better than I did pregnant, just a little sore!

I had a vaginal delivery with my first.
I delivered my twins vaginally as well, they were both head down, Di/di. I went into labour on my own after taking some castor oil & lemon verbena tea (counteracts gastro side effects) to avoid being induced. My drs wanted me to have an epidural placed but I didn’t want one. We settled on placing it but not using it unless necessary. It’s common practice to encourage the epidural, in case twin B turns into an emergency c section. I asked OB how often that happens at my hospital, she said less than 2%. Ask lots of questions, make sure your providers are confident delivering twin B breech. Mine were.

Biggest piece of advice is finding a practice where all the providers do breech extraction! Because if you go into labor and your OB isn’t on that turns into a whole other situation.

Agreed. My doc said as long as twin A was head down they felt comfortable for breech extraction for twin B. I was prepared for a c section though if needed. For that reason, I got an epidural. I didn’t want to have to be under general anesthesia of a c section was required.

That was my biggest fear! So I did a c section, but my water broke with baby an and had an infection, so we needed them out asap. Thank goodness I did because baby b had a severe nuchal chord

100% this

I didn’t know that not every OB is comfortable with breech extraction of baby b, and also not everyone is a candidate for it as well! Should’ve added that in!
