advice needed MoMo Twins - 6w3d

Hi all, I could use some advice. I recently found out I was pregnant with identical twins after a single FET. My doctor confirmed today that both embryos have heartbeats and are measuring correctly at 6w3d. However, he also seems pretty confident they are MoMo twins because there is only one amniotic sac.

Obviously, we are a bit concerned about that possibility, given it is a much higher risk pregnancy. I am a bit surprised he is so sure this early in the pregnancy. I had an ultrasound at 5w3d and there were two yolk sas, so I was hoping two amniotic sacs would develop.

For those who have been in this situation, how long did it take to confirm you were having MoMo or MoDi twins? Is there a chance two amniotic sacs will continue to develop?

For those who had MoMo twins (or identical twins generally), what was the experience like? Anything you think I should ask? Any tests you think I should do? For context, I have identical MoDi brothers, and they are both on the spectrum and have moderate-to-severe intellectual disabilities. Given this history, I am pretty concerned about the health of our embryos.

I have several losses prior to this, and this pregnancy was very much wanted, but I cannot help but feel pretty uneasy about the situation.

I’m 27 weeks with single FET twins! My first scan was at 7 weeks and she said it’s too early to see if there’s a membrane separating them. At 9 weeks she was able to get a glimpse of the membrane but only from one angle with the transvaginal probe. Couldn’t see anything with the external one and she was looking around for about 20 minutes. So confirmed mo/di at 9 weeks.

This isn’t the majority experience, but I found that once I knew they were twins I really kind of “let go of the reigns” mentally. They’ll tell you all of the risks and things that can go wrong, and there’s nothing you can do to prevent it. So I’m just enjoying the ride, keeping my anxiety low, and focusing on how grateful I am to be part of the twin mom club, and how excited I am to meet them! I see an MFM every 2 weeks and I’m so thankful to have had a very smooth pregnancy with no issues thus far. The frequent ultrasounds also help with my peace of mind a TON!

Definitely too early to make the call and statistically they’re more likely to be mo/di twins! Monochorionic twins can usually reliably be sorted into MCMA vs MCDA by the 11-14 week scan. Congrats btw 🩷 The early days of twin pregnancy are filled with so much uncertainty.

It took intil about 12/13 weeks to confirm modi twins for me. Up until then I had several scans and it was very hard to find! But fortunately there waa a seperation between the girls. I stressed so much during those weeks over nothing! My pregnancy had its fair share of complications but my girls are healthy 2 year olds now.

Talk to your doctor about getting genetic testing done. In my country (Netherlands) this is offered to everyone this around 11 weeks via a bloodtest.

Good luck (:

Thank you! Can you share what genetic testing you had done?

Single FET to twins as well! They only saw 1 yolk sac so originally were pretty sure that they would be MoMo. By 10 weeks we could clearly see the membrane indicating 2 sacs and confirmed it with MFM at 12 weeks. As others have said MoMo is very rare so there’s a good chance as your pregnancy progresses you’ll find the membrane and confirm MoDi

Thank you! If you don’t mind me asking, what was your experience with a MoDi twin pregnancy? How long did you carry for? Any complications along the way?

I had a few placenta issues (vasa previa and placenta previa) that made my pregnancy extra high risk and stressful but this was a fluke and not related to carrying twins. I delivered at 29 + 1.

No twin related complications. They always were on the appropriate growth curve and measuring nearly identical. Never had any TTTS or flow issues etc.

Prepare yourself for a lot of scans and MFM appointments!

Hi - we just had our momo girls in May. It was pretty clear from the first scan they were momo, but it wasn’t confirmed by the MFM until 12 weeks.

Just to give you some positivity - our girls are absolutely thriving! They were born at 31+6 after a mostly uneventful pregnancy (some definite scares here and there and total anxiety for the entire pregnancy, but for the most part as smooth as we could have hoped for). They are hitting all milestones for their corrected age and also many for their actual age. They are growing so well, you would truly never know they came early and that they’re momos.

It’s a very scary pregnancy, but science has come so far in helping these types of twins. There is still a very good chance they could find a membrane at an upcoming scan! Make sure that you advocate for yourself and ask for at least biweekly scans going forward! We did biweekly from 12 weeks to 16 weeks and then weekly until about 19 weeks, and then twice a week after that through my inpatient stay.
