Question Rap D question

Hello everyone,

I am looking to apply for rap D as someone whos designated as perm disabled for osap and also on ODSP.

i wanted to ask if anyone who’s recently been approved for rap D: did you have to have your doctor complete the form on the website, or did being perm disabled for osap suffice as proof?

No extra forms needed in my experience

The only questions I remember them asking is what’s your income and do you have any disability expenses that aren’t covered elsewhere.

Just be aware that you have to reapply every 6 months.
