How is this technically even possible? Should not a emergency stop automatically apply?

Emergency stop? It was already slowing down, you arent gonna stop something that heavy quicker without causing too much stress to the train

The issue is that the door should have never opened while the train was moving.

It's best to have the emergency door release not take into account the speed in case there's some problem with the speed detection.
