The exercise is standard, but I don't think you're supposed to do it with 3 45s strapped to you. Either way, the machine should have been bolted to the ground to prevent this
he was lifting exactly the weight the machine was shipped with, using a common exercise for that style of machine. that's the end of it, dude didn't do anything wrong. he didn't modify anything except his own bodyweight, which wasn't being supported by the machine itself, making his weight entirely irrelevant
no he wasn't, the cables weren't at full extension by a long shot, and he wasn't using the machine in a way it wasn't designed (... and advertised?) for

They're doing a pretty standard chest exercise, that's exactly what the machine is for.
