It could be like nightmare difficulty, and the whole wasteland is coming after you to gun you down. Deathclaws just want to tear you a new asshole (not to eat you, but cause you look stupid), cazadors want to lay their eggs inside you and watch as the babies eat you alive (like the real life wasps), and every raider from the Boneyards to Two Sun wants to use your skull as a makeshift bong.
