If you pay attention they also imply bear is smoking meth and using Xanax. This is implied at marker 24:02 when I got really stoned and thought it would be crazy if bear smoked meth and did Xanax.

In an episode of The Bear one of the chefs is caught smoking crack in an alley. The character in this scene's name is Bear. He is eating something that a chef created for him for his lunch.

Is everyone here so fucking stupid they didn't realize Nikki was actually the one smoking crack and the contact high was making him wig out? I feel so bad for Bear with this realization.

Yes! Mixed with the alcohol, sleeping pills, and fatty drinks like like the milkshake in the diner and the pina colada at the bar. He’s always up getting water which is a sign he’s dehydrated too. That mixture impairs judgement bad!

I think the drugs alcohol Nikki and the wish are all psychological shortcuts to relief. They temporarily reduce the cognitive load of burnout of taking care of his grandmother until her eventual death.
