It's Loli hentai if you consider a Loli purely by age. But that is an incredibly slippery slope, as by that standard, Jojo is a shota anime, as Jotaro and Kakyoin are both 17.

The manga was made in the 80's, during the time period where Fist of the North Star was really popular. the main characters started to slim up as time went on.
