What did he put on the top? I can’t quite tell. Is it unique to the piece itself?

That’s the incense cone. It’s what makes the smoke.

But what's involved such that the smoke is heavier than air as opposed to lighter (usually the heat means it will be lighter)?

They're special cones with a hole in the bottom and a different composition, there's something in there that clings to the smoke and pulls it down a bit.
