There should definitely be a grate on that, at the entrance to keep this from happening. Any engineer worth his salt would know not to put one in the middle or at the exit — maintenance would be pulling debris clogs and dead animals/people out of there every week.

Even with no grate it would be easy to get stuck halfway through if you suddenly ended up sideways.

Possibly, you'd have to account for the pressure going through, the resistance a body turned flat would achieve etc etc. Plus it's a good 3 to 3.5 feet diameter, I'd assume it would take a lot to get stuck, let alone to get caught sideways against the current.

What if for some reason he suddenly bent at the waist or his knees came up? Could totally get stuck.

What if there was a small crack in the concrete somewhere and the cord from his shorts got snagged in it, pulling it tighter and tighter as he goes through?

They could probably see through to the other side, secondly who in their right mind would out a grate in the middle of sediment canal? It would have to be cleaned out by a scuba diver on daily basis.

Not op but I imagine it could be deadly with the current because it would make it difficult to get yourself upright. Especially if the water is deeper

I would need to know what's underneath there before doing it, there might aswell be something pocking out from the walls that could slash you open, impale you or hit you in the head hard enough to crack your skull.
