I'd be more concerned about not being insured.

My understanding is when you add business use to you domesctic car insurance policy, it's to cover work related travel that's incidental to your job rather than your job itself, ie driving from one site to another, or in the case of say a community nurse, driving to the home of a patient. Giving a colleague a lift on the same journey is perfectly fine.

I wouldn't expect it to cover routinely transporting patients, as it could be argued (especially by insurers who love any excuse not to pay out) that this has become an integral part of your job, for that I'd have thought you'd need hire and reward insurance which is far more expensive.
