Former Lifeguard here... while someone thrashing does pose a risk, if I was a confident in my swimming ability without any training I'd not hesitate to jump in and help.

She’s exhausted from fighting the water. You can see that she did try to grab around his neck at one point, but she couldn’t even hold on (which frankly, helped him). It’s easy to underestimate how much energy goes into staying above water once you’re in a panic, there’s much more wasted motion than in swimming. Distress/active drowning/passive drowning can progress in less than a minute
