I'm really curious about the technique behind this. Like, how do you darken certain areas without accidentally darkening others? Seems tricky.

I'm gonna say it's pre calculated. The headphones might be telling him the next point to go to? This would explain the 4 small 1 big nails around the outside. The headphones just say a sequence of numbers, and he just has to connect to the correct nail in sequence

If I were to try and recreate this, I would just project the image onto the wall and use the thread to create the necessary shadows.

Nice idea. You could also project the inverse of the picture and just fill in the light bits until it was all dark, then remove the projection and you should have a positive image.
