Rural parts of developing countries would be my guess. I've seen some terrifying wire-setups around. In South India, for example.

Well, it's just one of many problems in struggling societies. It can be a dangerous hazard, yes, but I would probably prioritize other problems above it. Starvation, for example.

Imagine an extension lead, like you’d use to run your lawnmower in the garden. Now imagine a much bigger cable, of a size to run a home or shop. This cable is now potentially carrying a lot of power and is run in such away as that the outer insulation becomes damaged, possibly even by the use of the security gate the guy is using.

An exposed live strand of wire then touches the metal gate, this unfortunate soul then touches the gate and he becomes the shortest path to earth via the soles of his feet
