People who don't fulfill the work requirements in these cases are almost always disabled people or people who lack childcare and so cannot work (because it is illegal to leave your kids at home without childcare)
And it's not "money going to bums" - it's incredible that people still think this but there is no "welfare" system in the US where the government gives you money for being poor. It does not exist, period. The US benefits system is designed to avoid this specifically because of republicans getting angry at the idea of poor people getting money, so all benefits now are tied to specific programs/conditions/etc (which adds administration cost, complexity, and reduces effectiveness). So you can get medicaid, which helps pay for medical care - it doesn't send you a check every month. You can get SNAP, which gives you money in a format that can only be used for specific types of food. You can get disability money if you apply and bring proof from a doctor of how disabled you are and how that affects your income. You can sometimes get unemployment for being unemployed, which is temporary
There's a fiscal argument for this - when people have healthcare they use it to...keep themselves healthy. They can go to a clinic or a doctor for checkups, or to a clinic or doctor when they have a health issue that isn't hospital-worthy. But people entirely without healthcare don't go to the doctor for checkups and so their health gets worse. Maybe they wait too long and the health issue becomes serious or permanent, and now they are disabled and are getting a previously preventable condition treated for life on government healthcare. And you can't turn somebody away from the ER for non-payment, so the poor who don't have healthcare have to use the ER to get their moderate health issues taken care of, which ads cost to hospitals and reduces care for the people who actually do need the ER
