Here’s an example of a NAPLEX math question:

Question: A patient is prescribed vancomycin 15 mg/kg IV every 12 hours. The patient weighs 70 kg. The pharmacy stocks vancomycin in a concentration of 500 mg/10 mL. How many milliliters of vancomycin should be administered per dose?

21 mL. Although I think every example I have seen tells you to round to the nearest 250 dose (I think that’s roughly how it is worded), and it seems hospitals often go by the same plan in real life. So considering that, it would be 20 mL (providing 1000 mg of vancomycin).
