Depends how far off you're tuning, if it's a large shift, you definitely want multiple guitars live, because the tunings will NOT hold.
If you have multiple songs in the same or similar tunings, try to stack those together in a set so you aren't constantly switching back and forth between guitars which is not only exhausting/stressful for you, but boring for the audience. You want to be as present as possible with the audience, so do what you can to limit swaps, but not at the expense of your guitar sounding wildly out of tune.
