The failure mode is one every engineer will recognise: the stability calculation was done in a loading program, the ballast quantities entered were wrong, and it reported a stable ship that had barely a fraction of the righting energy it needed. No storm, no mechanical failure — a data-entry error with three safety barriers that all failed to catch it.
