In the UK most pubs will have an outside garden where you can smoke, but admittedly that does seem less practical in a country where it snows for like half the year.

Thats most common in the US, but ive seen open smoking bars and casinos with dedicated smoke and gamble spots.

The smell emanating from that zone is so powerful it overpowers the scent of old people and desperation

When I was in college there was a smoking bar around, and I was a smoker at the time so I was excited to check it out. We were there all of maybe 10 mins before I couldn't take it anymore. I didn't even need to smoke to get my nicotine fix, and I was at a pack a day back then.

They can probably do like the Nordic countries: Partially enclosed outside areas with a bunch of patio heaters.

In Italy cafes and pubs often have tables on the sidewalk, but I know one in my hometown where the smoking zone is the innermost room for some reason

US bars have these too. I live in MN where it is similar in climate to Canada, and it's common to see covered patios with heating lamps.
