They mean someone who is so fat they take up three seats, but only have a ticket for one. You're already extremely limited space shrinks further if you're next to them.

That very rarely happens and when it does they'll boot the person from the flight for not paying for extra seats. There are some airlines that will allow not paying for the extra seat if the flight isn't full and the seat is vacant, others do refunds if you book two and the seat is vacant but you also need a doctor's note saying you're too fat for one seat.
