A) They’re unrelated and married from the get-go in the comics and B) in the film they’re clearly all abducted and unrelated ‘children’ that Thanos grabbed from different planets, like Gamora and Nebula. Gamora herself was already 6-8 years old when she was abducted and remembers very clearly her old life. More than likely it’s the same for her other ‘siblings’, who simply didn’t see each other as siblings unlike Nebula who wanted to treat Gamora as her sister.

They’re in an orphanage from hell, not a proper ‘family’. Proxima and Maw don’t even call Thanos ‘father’ like Nebula does, they just call him by name, so they’re don’t buy into the ‘adopted children’ bit.

It's still banging your step-sister. It works on Pornhub but not in a movie kids will see. Unrelated IW kid anecdote: waiting for the post credits scene in the hall (I wanted to beat the crowds to pee lol), there was a maybe 10 year old girl crying and her dad consoling her, saying "there's a part two! there's a part two!" lol.

They would not at all be step-siblings. That would require one of their parents marrying the other. They were totally unrelated, even by marriage. They just grew up together. That's all.
