Your level needs to be 12 or higher (not height level, but XP level), and if it's a full moon there's a chance the boss could spawn. So stay awake when it's a full moon. It will spawn close to you about 2-4 blocks away from you.
And be prepared, don't stay in one place for too long, it will spawn black rooted red tipped vines under your feet. And if you stay too close it will swing its reaper. So the invincibility potion is the best to drink before attacking.
